NOTES:

• ONSITE RESEARCH ACTIVITY SHOULD BE KEPT TO A MINIMUM and include those activities that cannot be done remotely and are essential to your research program.
• Principal Investigators who previously received a Stage 1 research exemption are not required to complete the Stage 2 intake form. Updates to personnel for these previously approved exemptions can be submitted directly to BCCHR Facilities via email: tjacques@bcchr.ca
• For Principal Investigators who did not previously receive a Stage 1 research exemption, or if the nature of the work proposed for Stage 2 is distinct (research activity or area), then a new form should be submitted.
• If the nature of research activities is similar or related, it can be submitted in one form. If the nature of the work is distinct (research activity or area), you can submit multiple forms.
• Requests for one-off site visits should be sent directly to BCCHR Facilities via email: tjacques@bcchr.ca.
