Demographic information, including information on ethnicity, will be used to understand who is interested in our programs and will be shared with the BC Children's Hospital Foundation donors to promote the value of initiatives like this one. We will only share information about the group as a whole, and will never share any details about you personally outside of this program.

Your personal information is protected by our privacy law in BC (called the Freedom of Information and Protection of Privacy Act (FIPPA)). For this form, your personal information is collected under section 26 (c) of FIPPA. We will only use your information for the purposes listed on this form. Only the BCCHR Research Education & Services Team and the technical support team at BC Children's Hospital Research Institute will access your information. The project team will launch the form and analyze the results. Form data will be stored indefinitely by PHSA and BC Children's Hospital Research Institute in their Secured Networks electronically. Participants will be contacted directly regarding their registration status.
